1. Your Trading Partner ID is in the email that CEDI sent to you.
2. Type of Trading Partner: Direct billing
3. Your Trading Partner name is your supplier name as it appears on your 277 acceptance reports
4. You’ll list your NPI and your ptan/supplier number. The NPI is also listed on your 277 reports.
5. Your supplier number is your ptan for Medicare DMERC.
6. Primary Contact should be the provider
7. Office staff can be additional contacts
8. The Signature should be the provider’s
9. CEDI will process the form and send you an email in a few days to let you know if it was accepted or not. If it was not accepted, you will need to re-submit it with the correct information.
