During the upgrade process, all of the previously entered notes were locked. This means they cannot be edited by anyone aside from the user or provider who created the note. 

When you save your note, just clicking close does not sign the note. You will need to sign 30 percent of your notes for patients that you billed an E&M code for at that visit in order for you to pass the menu measure for Stage 2.
