If you see in claim history green lines with different insurance company or patient names (and that pattern exists for that patient's entire claim history and for other patients who had the insurance company that patient used to have even though the patient chart still shows the correct insurance company), then this is account number record number change.
To rectify this, please contact support immediately. We will require a backup of your system and no insurance entries can be added to the library until the issue is resolved. This is something that can be corrected by the support department easily and will be corrected within 24 hours.
